Pharmacogenetics in Practice: Drug Management Amidst Rare Genomic Testing

چکیده

Although personalized medicine is an ideal concept, there remains little training or discussion to its applicability for the primary care provider. Psychiatric, cardiac, gastrointestinal, and pain are common complaints/conditions seen in care, treatment options have genomic implications. This article provides experienced clinician with background, clinical utility, of pharmacogenetics medications commonly prescribed such as selective serotonin reuptake inhibitors, tricyclic antidepressants, beta blockers, clopidogrel, statins, proton pump codeine. The dose adjustment alternative considerations based on a patient’s pharmacogenetic profile discussed.

Pharmacogenetics, drug-metabolizing enzymes, and clinical practice.

The application of pharmacogenetics holds great promise for individualized therapy. However, it has little clinical reality at present, despite many claims. The main problem is that the evidence base supporting genetic testing before therapy is weak. The pharmacology of the drugs subject to inherited variability in metabolism is often complex. Few have simple or single pathways of elimination. ...

Pharmacogenetics in Practice Pharmacogenetics in Cardiology: Forays into Modern Practice

When one discusses pharmacogenetics in cardiology practice, it is almost obligatory to discuss clopidogrel. Clopidogrel is an antiplatelet used for acute coronary syndrome (ACS), percutaneous coronary intervention (PCI) with stenting, and as secondary stroke prevention in patients with aspirin allergy, among other indications. Clopidogrel is a prodrug that must be metabolized through the cytoch...

Pharmacogenetics and Predictive Testing of Drug Hypersensitivity Reactions

Adverse drug reactions adverse drug reaction (ADR) occur in approximately 17% of patients. Avoiding ADR is thus mandatory from both an ethical and an economic point of view. Whereas, pharmacogenetics changes of the pharmacokinetics may contribute to the explanation of some type A reactions, strong relationships of genetic markers has also been shown for drug hypersensitivity belonging to type B...
